Telehealth Patient Questionnaire
Your completion of our intake questionnaire is required prior to scheduling your new patient appointment. The initial consultation will require 30 minutes.
During the initial consultation you will meet with Dr. Voisine via video chat (Zoom). In addition to your medical history he will review options including medications and Health Coaching programs (Anew10, NOOM). You should receive an email within 24 hours of submitting this questionnaire.
The email will include appointment times for you to meet with him. Additional information will be sent to you once the appointment has been confirmed.
